Health Equity Moments: Screening for Social Drivers of Health at Duncan Hospital

Hear from Jodie Franzen, located at Duncan Regional Health in rural Oklahoma, as she highlights the organization's proactive approach to screening for social drivers of health (SDOH) to reduce readmission rates. Already implementing screening questions before new requirements emerged, Jodie and her team utilize the PREPARE tool to address issues like personal safety, food insecurity, and health literacy. Staff conduct screenings during patient admissions, supported by training and a "We Ask Because We Care" campaign to explain the purpose to patients, who have responded positively. The organization connects patients with resources, including a food bank, and evaluates effectiveness through data collection and community feedback, particularly focusing on the needs of the Hispanic population. Jodie notes initial hesitance from staff in asking sensitive questions but emphasizes their commitment to enhancing community health outcomes and ensuring equitable access to care.

Health Equity Moments: Health Equity and Aging in the LGBTQ+ Community

In this episode, Telligen & ATW hear from SAGECare [https://sageusa.care/] team members Nic Watson (he, him), Managing Director of Social Enterprise and Sadiya Abjani (she, her), Director of Training and Instructional Design. SAGECare is the country's oldest and largest nonprofit dedicated to improving the lives and health outcomes of LGBTQ+ older adults. The organization was founded in 1978 in New York with the idea that "if nobody takes care of us, we must take care of each other." SAGECare provides LGBTQ+ competency training and consulting on LGBTQ+ aging issues to service providers in the U.S. and internationally. SAGECare offers the added benefit that qualifying agencies may receive a national credential to highlight the percentage of staff trained.
