The Pelvis Isn’t Frozen: Why Millimeters Matter

She explains that the pelvis is neither rigid nor wildly unstable, but moves in small, meaningful ways affecting pelvic floor function, pressure management, and force transfer. Dr. Becky distinguishes pelvis-on-femur motion (pelvis rotating over a relatively fixed femur in gait, squats, lunges, etc.) from innominate/ilium rotation and from sacral nutation/counternutation, describing how these related motions influence hip external rotators and pelvic floor length-tension. Ilium internal rotation is linked with pelvic outlet widening and posterior rotation/extension, while external rotation narrows the outlet with anterior rotation/flexion. She also clarifies that mobile pelvis models exaggerate motion for teaching, but millimeters of SI joint motion still affect symptoms such as leaking, heaviness/prolapse, pain, hip pinching, bowel mechanics, and birth mechanics. In this episode of From the Ground Up, Dr. Becky Allen explains that joints—and the pelvic floor—need three essentials to function: adequate range of motion, control of that range (especially eccentric control while lengthening under load), and strength/power to recoil and move out of the loaded range (concentric propulsion). Applying this to pelvic health, Dr. Becky argues the pelvic floor is not simply “tight” or “weak,” but must lengthen, load, recoil, and manage pressure in coordination with the pelvis, ribcage, feet, hips, and the entire kinetic chain. Limited pelvic motion (internal/external rotation, expansion/compression) reduces pelvic floor options, contributing to symptoms such as heaviness, pressure, prolapse, leaking, tailbone/back/hip pain, and painful intercourse. In this episode, Dr. Becky Allen argues that posture should be assessed as a signal of a patient’s movement, balance, and breathing strategies rather than treated as the problem. She critiques common assumptions about anterior and posterior pelvic tilt (including “Lower Crossed Syndrome”) and explains that the nervous system drives posture, with the rib cage influencing pelvic orientation and pelvic floor function. Two common patterns are described: “Big Booty Judy” (anterior tilt with flared, inhaled rib cage and short, descended pelvic floor) and “Pancake Butt Paul” (often an underlying anterior tilt with compensatory glute clenching, forward pelvic translation, pump-handle down rib position, and even more pelvic floor pressure).
