A client's lower back keeps flaring up, so you treat the back. But the actual source is limited hip rotation, and the back is just the next joint in line taking the strain. Dr. Sean Altman explains how coaches learn to trace pain to where it's actually coming from. In this episode of the FitBiz Accelerator, host Tobias Young sits down with Dr. Sean Altman, a doctor of physical therapy and functional medicine doctor specializing in sports and regenerative medicine. Drawing on 20+ years as a personal trainer and his own history of injuries and chronic pain, Dr. Altman shares the approach he uses to help people reverse chronic pain, restore function, and avoid unnecessary surgeries. He breaks down the four connected systems behind his upcoming book, The Altman Method: structure, chemistry, the nervous system, and cellular energy. You'll hear why persistent tightness is often a muscle guarding an injury rather than a stretching problem, and why a client who seems weak may not have atrophy at all. Sometimes the brain caps a muscle at 60% output because of pain and inflammation, so no amount of pushing gets them past it. Dr. Altman also uses a simple battery analogy for recovery. If sleep and nervous system regulation aren't letting the body recharge fully, your client starts each week at 80%, then 70%, until they hit a wall that looks like a plateau but is really an energy deficit. And for online coaches who can't assess clients in person, he explains what you can still catch through feedback alone, like a client reporting that a back feels worse the day after a stretching session.
