Here's how to navigate the DOAC jungle. ā When DOACs Are Preferred * Venous Thromboembolism (VTE) * Atrial Fibrillation * ā ļø Exclude patients with: * Mechanical heart valves * Rheumatic mitral valve disease Ā (Thatās why cardiologists note ānon-rheumaticā AF in their documentationātreatment plan hinges on it.) ā When DOACs Are a No-Go * Mechanical Heart Valves ā Warfarin only * Rheumatic AF ā Warfarin still rules * Thrombotic Antiphospholipid Syndrome ā Warfarin * Transcatheter Aortic Valve Replacement (TAVR) ā Antiplatelet therapy * Embolic Stroke of Undetermined Source ā Antiplatelets preferred š¤ The Gray Zone: Uncertain Use Cases These arenāt absolute yes or no. For classic AF and VTE? Go for it. For valves, rheumatic disease, or complex syndromes? Tread carefully.
