Brain fog can be one of the most unsettling changes in midlife because it is hard to explain and easy for other people to dismiss. You might lose your train of thought mid-sentence, search for words that used to come instantly, or feel like your mind is moving through wet cement. I want to make this simple and validating: brain fog is common, it often comes and goes, and normal labs do not automatically mean nothing is happening. I break down what brain fog is and what it is not, then map the most recognizable patterns I see in clinical practice: sleep-deprived fog, blood sugar fog, hormone transition fog in perimenopause and menopause, inflammatory or histamine-related fog, thyroid or mitochondrial slow fog, and the overloaded nervous system version where you are wired but tired. Once you can name your pattern, you can stop guessing and start choosing the right lever. From a women's health and functional medicine perspective, I walk through the highest-yield root causes to screen, including sleep architecture, protein and underfueling, glucose variability that does not show on A1c alone, estrogen and progesterone shifts, thyroid signaling, ferritin and B12 status, inflammation and gut health, and hidden contributors like alcohol, dehydration without electrolytes, certain medications, and supplement overload. You can be “fine” on paper and still feel wrong in your body. If you’re getting through the day, exercising, eating pretty well, and your labs look normal, but you feel flat, foggy, reactive, or stuck, we want to offer a better frame: under-recovery. I’m Dr. Sheri Erwin, an integrative nurse practitioner, and I’m walking you through why this shows up so often for high-performing women in their 40s, 50s, and beyond and why more discipline is not the answer. We break down what under-recovery actually is: when total stress exceeds your current recovery capacity. That stress includes workouts, work, caregiving, sleep disruption, travel, undereating, inflammation, illness, and the hormone shifts of perimenopause and menopause. We walk through what systemic hormones actually mean (hormones circulating in the bloodstream) and why they can be so effective for whole-body menopause and perimenopause symptoms like hot flashes, night sweats, sleep disruption, mood changes, brain fog, and joint pain. We also clarify a common online blind spot: if you still have a uterus, progesterone is often part of the plan to protect the uterine lining.
