Low Energy, High Functioning: When You're Not Tired, You're Under-Recovered

You can be “fine” on paper and still feel wrong in your body. If you’re getting through the day, exercising, eating pretty well, and your labs look normal, but you feel flat, foggy, reactive, or stuck, we want to offer a better frame: under-recovery. I’m Dr. Sheri Erwin, an integrative nurse practitioner, and I’m walking you through why this shows up so often for high-performing women in their 40s, 50s, and beyond and why more discipline is not the answer. We break down what under-recovery actually is: when total stress exceeds your current recovery capacity. That stress includes workouts, work, caregiving, sleep disruption, travel, undereating, inflammation, illness, and the hormone shifts of perimenopause and menopause. You’ll hear the most common “looks like real life” patterns, including wired but tired, low motivation without feeling depressed, stubborn central weight gain or puffiness, and soreness that lingers longer than it used to. We also cover the six drivers I check first: sleep quality and circadian rhythm, training intensity without real recovery architecture, underfueling (especially protein and carb timing), hormone transition and HRT changes, adaptive thyroid signaling shifts, and hidden inflammation or nutrient shortfalls like low ferritin or low B12. To make this actionable, I share a simple seven-day recovery audit you can do without testing, plus a seven-day reset that often moves the needle fast: one true recovery day, protein anchors, morning light, a noon caffeine cutoff, an earlier dinner, and electrolytes if you train hard. Anxiety can be real and still be the wrong diagnosis for what you’re feeling. If you’ve noticed a sudden shift in stress tolerance, sleep, mood, or feeling “wired” at night, we walk through a different lens: anxiety-like symptoms as stress physiology. I’m a nurse practitioner and integrative women’s health educator, and I want you to understand what your nervous system might be trying to communicate before you assume you’re broken. We dig into cortisol dysregulation and the tired-and-wired pattern, including why your body can feel exhausted all day yet flip on the moment your head hits the pillow. You’ll learn how cortisol is supposed to follow a daily rhythm, how modern stress breaks that rhythm, and why elevated evening cortisol can suppress melatonin and trigger 2 a.m. or 3 a.m. wake-ups with a racing mind. We also connect the dots on why symptoms often worsen after poor sleep, too much caffeine, or seasons of burnout. Then we get specific about hormones and metabolism. We explain how estrogen affects brain signaling and sleep regulation, and why progesterone is often the missing piece in perimenopause anxiety. Progesterone supports GABA, your brain’s primary calming pathway, and when progesterone declines many women lose some built-in nervous system “brake” support. Finally, we cover blood sugar instability and why a glucose drop can trigger adrenaline and cortisol that feels exactly like panic, plus practical steps like a protein-forward breakfast, steadier meals, and backing off caffeine as a substitute for fuel. If Hormones Did Not Help You, You May Have Tried The Wrong Kind

Most women hear “HRT” and assume it’s one uniform treatment. That single label hides a crucial truth: systemic hormone therapy and local vaginal estrogen are different therapies, used for different symptoms, in different tissues, with different goals and expectations. We walk through what systemic hormones actually mean (hormones circulating in the bloodstream) and why they can be so effective for whole-body menopause and perimenopause symptoms like hot flashes, night sweats, sleep disruption, mood changes, brain fog, and joint pain. We also clarify a common online blind spot: if you still have a uterus, progesterone is often part of the plan to protect the uterine lining. Along the way, we talk about why hormone therapy is always individualized and depends on personal health history and risk factors. Then we shift to one of the most under-discussed topics in women’s health: localized vaginal estrogen. We explain how estrogen receptors in vaginal and urinary tissue affect dryness, painful sex, burning, urinary urgency, bladder irritation, and recurrent UTIs, and why repeated antibiotics may miss the underlying tissue changes of genitourinary syndrome of menopause. The key takeaway is simple but powerful: the “right” hormone therapy depends on the symptom you’re trying to treat, and many women need a more targeted approach than they were offered. Congrats On Your Normal Labs. Now What?

“Your labs are normal.” If you’ve heard that and still felt exhausted, foggy, achy, moody, or like your body is changing without an explanation, we made this for you. Normal bloodwork can be genuinely reassuring, but it can also be incomplete because standard lab panels are built to detect disease, not to optimize health span or explain early dysfunction.  We walk through the biggest places “normal” can hide the real story, starting with thyroid labs. If you’ve only ever been offered TSH, we explain why Free T3, Free T4, and thyroid antibodies can matter, especially when symptoms like cold intolerance, constipation, hair shedding, or low mood show up. Then we shift to metabolic health and the early warning signs of insulin resistance that don’t always appear on fasting glucose alone, including fasting insulin, hemoglobin A1C trends, and the triglyceride-to-HDL ratio for cardiometabolic risk.  We also cover inflammation markers like high-sensitivity CRP and homocysteine, plus nutrient labs that can change how you feel even when a CBC looks fine, including vitamin D, ferritin, B12, and magnesium. And for women in their 40s and 50s, we talk about why reproductive hormones may be worth exploring when sleep, weight, libido, and mood start shifting. I walk through what’s changing under the hood, including estrogen decline, reduced insulin sensitivity, blood sugar swings, and the quiet but powerful impact of losing muscle over time. We also connect the dots between chronic stress, elevated cortisol, and stubborn abdominal fat, plus how disrupted sleep can raise hunger hormones, lower satiety, impair glucose tolerance, and keep your body stuck in a “perfect storm” of cravings and fatigue.
