About The Episode: On this episode, Marc discusses that a combination of worrying drug trends, MA growth, and political interference has undermined the Part D program's stability. Key Takeaways: The Part D program was designed to be a narrow margin program with risk-sharing with the federal government. Rising drug costs and utilization as well as increased enrollment in Medicare Advantage (MA) have made the program riskier for plans along the way. Democrats then passed cost-sharing reduction to gain political points in 2022 and 2024. The combination of the two has deeply undermined the financial stability of the Part D program, increasing risk to plans and costs of the program. MA has the advantage of being able to subsidize Part D from Part C revenues, but standalone Part D (PDP) plans cannot. They are in the worst shape. All this has led to less choice, premium and cost-sharing increases for many, and narrower benefits. While the federal cost-sharing changes aimed to improve affordability, it has also introduced new forms of cost shifting—many of which are less visible, but no less real. The federal government had to introduce a premium stabilization program to stop further surges. Lawmakers and CMS will have to recalibrate and restructure the program to preserve it. Marc is an unconventional Republican who believes that affordable universal access is a wise and prudent investment. He recommends common-sense solutions to reform American healthcare. Tune in every week as Marc examines the latest developments in space, offering analysis, insights, and predictions on the changing state of healthcare in America. About The Episode: On this episode, Marc discusses his 5-part series on healthcare reform as the 2026 midterms approach. Key Takeaways: If we could redesign American healthcare from the ground up, what would it actually look like? The time has come for a grand bargain as employers, consumers, and government can no longer shoulder the huge healthcare costs burdens. America has a fragmented healthcare system that is terribly inefficient and costly. We have the most expensive system and get the least in quality outcomes. Over these five weeks of the series, we will explore three pillars of reform: --Price reform – it is about price and not necessarily utilization. --Pivoting to care management to emphasize primary care and prevention. -- We need to fashion true affordable access. Democrats and Republicans on Capitol Hill should come together as each side could gain something they want. 132. A Medicare Advantage Stars Earthquake: CMS Reforms, the Clover Court Decision, and What Comes Next

A recent CMS regulation and court decision means great changes for the Medicare Advantage Star program. About The Podcast: Millions of Americans feel confused and frustrated in their search for quality healthcare coverage. Between out-of-control costs, countless inefficiencies, a lack of affordable universal access, and little focus on wellness and prevention, the system is clearly in dire need of change. Hosted by healthcare policy and technology expert Marc S. Ryan, the Healthcare Labyrinth Podcast offers accessible, incisive deep dives on the most pressing issues and events in American healthcare. Tune in every week as Marc examines the latest developments in space, offering analysis, insights, and predictions on the changing state of healthcare in America. About The Episode: On this episode, Marc discusses the seismic changes to the Medicare Advantage Stars program from a recent CMS regulation and court decision. Key Takeaways: Seismic changes are coming to the Medicare Advantage Stars program. The recent CMS regulation massively streamlines the Stars program and focuses on clinical and survey measures. The easy-to-hit operational measures are being sunset. Between 25% and 41% of plans could see a reduction of at least one-half Star. A recent court decision in favor of Clover Health could reduce the number of measures from 34 to as low as 16. A judge said CMS does not have the statutory authority to include some measures and didn't follow procedures when including others. The case is likely to be stayed or overturned. If not, Congress will likely intervene as value-based payments are key to healthcare reform. Congress could make further changes, including making the program budget neutral and gauging quality at the local level. MedPAC, former CMS officials, and the influential Paragon Health Institute are all pushing for further reforms. MA plans need to redouble their efforts as Stars gets more complex and rigorous. 131. The ACA Exchange Rule, Mark Cuban, and the Beginning of a Healthcare Paradigm Shift

The just finalized 2027 ACA Exchange rule and a new proposal by Mark Cuban for direct primary care funding signals a paradigm shift in healthcare. About The Podcast: Millions of Americans feel confused and frustrated in their search for quality healthcare coverage. Between out-of-control costs, countless inefficiencies, a lack of affordable universal access, and little focus on wellness and prevention, the system is clearly in dire need of change. Tune in every week as Marc examines the latest developments in space, offering analysis, insights, and predictions on the changing state of healthcare in America. About The Episode: On this episode, Marc discusses the just finalized 2027 ACA Exchange rule and a new proposal by Mark Cuban for direct primary care funding. It signals a paradigm shift in healthcare. Key Takeaways: For years, healthcare policy has focused on one central goal: expanding comprehensive coverage. What if we've reached the point where coverage on paper is not the same thing as access to care? What if millions of Americans are technically insured but functionally disconnected from the healthcare system? And what if the future isn't simply richer insurance benefits, but a different way of organizing healthcare altogether? Various changes in the 2027 ACA Exchange rule promote a great deal of experimentation, from multi-year catastrophic plans to value-based insurance benefits to non-network qualified health plans. While controversial, the move aims to shake things up and think beyond broken insurance coverage -- which is increasing 6% to 10% per year and worsening affordability. Mark Cuban has proposed to redirect a portion of premiums subsidies to Direct Primary Care. The remainder would be used for a catastrophic coverage plan and put into a Health Savings Account. Economics, technology, and consumer sentiment are all forcing change and this kind of experimentation is key even paired with necessary price reform in healthcare. Health plan financials look up, but insurers are by no means out of the woods yet. Tune in every week as Marc examines the latest developments in space, offering analysis, insights, and predictions on the changing state of healthcare in America. About The Episode: On this episode, Marc discusses Q1 financial results for insurers. Health plan financials look up, but insurers are by no means out of the woods yet. Key Takeaways: A combination of external forces such as high utilization and policy impacts hit plan financials he past few years. As well, health plans suffered from a lack of financial discipline. The 2025 and Q1 1026 results show plans enforcing discipline and signaling conservative guidance. This has sent stocks soaring from recent lows. Plans' discipline meant retrenchment -- reducing benefits and choice as well as higher premiums and cost-sharing for consumers in all lines of business. But the recovery is not a slam dunk. Insurers will see tighter rates, further prior auth reform, and less enrollment due to Medicaid and Exchange changes. Vertically integrated insurers likely face less risks, while those with greater Medicaid and Exchange enrollment are more vulnerable. Consumers are paying more because somebody has to absorb the higher structural cost burden in healthcare. And over the next several years, we are likely to continue seeing a healthcare system defined less by expansion and more by managed retrenchment.
