Over And Then They Wondered

And Then They Wondered… is a space for the questions we don't rush to answer. Each episode explores the quiet tensions of being human — relationships, identity, growth, grief, creativity, purpose — and the moments that make us pause and think, "There has to be more to this." Through honest conversations and reflective dialogue, we lean into nuance over noise and curiosity over certainty. This isn't about having it all figured out. It's about learning how to wonder better.

TO DIE FOR: The Good Death

Death is one of the few experiences every human being will face… and yet every culture learns to meet it differently. In this episode, we explore the idea of "the good death" through the lens of cultures, traditions, spiritual practices, and modern perspectives from around the world. From ancient rituals to contemporary hospice philosophy, we ask: What does it mean to die well? What helps people face death with peace instead of fear? And what can different cultures teach us about grief, presence, love, and letting go? As hospice nurses, we've witnessed death at the bedside hundreds of times. But this conversation goes beyond medicine. Together, we explore the humanity of death — the beauty, the mystery, the rituals, and the wisdom hidden inside one of life's most universal experiences. Whether you are grieving, caregiving, spiritually curious, or simply learning how to live more fully in light of mortality, this episode invites you into a compassionate and deeply human conversation. Because perhaps learning how to face death… also teaches us how to live.

TO DIE FOR: What Hospice Medications Really Do

Hospice medications are often misunderstood—and sometimes even feared. In this episode, we gently walk through what hospice medications are, what they're used for, and most importantly… what they are not. As hospice nurses, we've had countless conversations with families who worry that medications might "hasten death" or take their loved one away too soon. The truth is, hospice medications are designed for one purpose: comfort. We break down the most commonly used medications in hospice care, including those used for pain, anxiety, shortness of breath, and restlessness. We also explain how these medications work in the body, when they're used, and how they help support a more peaceful, comfortable end-of-life experience. In this episode, you'll learn: * What hospice medications are and why they are used * The difference between comfort care and curative treatment * Common medications used for pain, anxiety, and breathing changes * How medications support—not speed up—the natural dying process * How families can feel confident and empowered in giving medications Whether you're caring for a loved one, working in healthcare, or simply want to better understand hospice care, this episode is here to bring clarity, peace, and reassurance. Because at the end of life, comfort matters most.

aflevering TO DIE FOR: When is it Time for Hospice? artwork

TO DIE FOR: When is it Time for Hospice?

One of the most common questions families ask is: "How do we know when it's time for hospice?" In this episode of To Die For, we walk through what actually qualifies someone for hospice care and the signs that it may be time to start the conversation. Hospice care requires a physician certification of a terminal illness with a life expectancy of six months or less if the disease follows its normal course. But eligibility is about more than just a diagnosis. It often becomes clear through patterns of decline that unfold over time. We discuss some of the most common indicators that someone may qualify for hospice, including: • General decline in overall health • Reduced functional status, often measured by the Palliative Performance Scale (PPS) • Unintentional weight loss or declining nutritional status • Increasing dependence in Activities of Daily Living (ADLs) such as bathing, dressing, eating, and walking • Frequent hospitalizations or emergency room visits • Persistent symptoms such as pain, breathlessness, or fatigue We also explore disease-specific guidelines that physicians consider when determining eligibility, including advanced cancer, dementia and Alzheimer's disease, heart failure, lung disease, stroke, and other progressive illnesses. Finally, we talk about some of the common referral signs families often notice first — frequent falls, increasing weakness, loss of appetite, recurrent infections, and a shift from independence to needing more daily care. Understanding these changes can help families recognize when hospice may provide the comfort, support, and dignity patients deserve in the final chapter of life. If you've ever wondered how to recognize when it may be time for hospice, this episode offers guidance, clarity, and compassionate insight into one of the most important decisions families face.
