Repetitive transcranial magnetic stimulation, or rTMS, sounds like science fiction: using magnetic pulses over the brain to change excitability, reduce symptoms, and potentially improve recovery after stroke. But is it actually ready for everyday stroke rehab, or is the evidence still ahead of clinical reality? In this episode of Neuro Simplified, we break down recent research on rTMS for stroke recovery, including motor function, cognition, gait, lower extremity rehab, spasticity, and central post-stroke pain. The research suggests rTMS may help with several stroke-related impairments, but the results are not simple. Studies show promise, yet many trials are small, protocols vary, and patient response is still difficult to predict. Hofmeijer and colleagues describe this clearly: rTMS has potential for motor and cognitive recovery, but the evidence is challenged by unexplained heterogeneity across studies. We also discuss why pairing rTMS with physical therapy may matter. One review describes how reducing upper limb tension may improve arm swing, trunk rotation, and gait coordination in people with hemiplegia, creating a better window for task-specific physical therapy. For lower extremity recovery, Qi and colleagues argue that rTMS is promising but may be limited by inaccurate targeting of the leg motor cortex, especially because lower limb representation is deeper and harder to stimulate precisely.
