We are victimized by chargemasters not tied to real costs as well as annual price hikes having nothing to do with real inflation. Consolidation has also eroded competition and driven price. America should have national or regional price setting from schedules or negotiations. This would reduce price in the commercial world and lead to major administrative savings. America should adopt site neutrality immediately to begin tackling price. Value, including value-based payments, must replace volume. Drugs, too, need price reform and the Trump administration has begun tackling this in a big way. International reference pricing has been adopted throughout the world and we should do so as well. The easy-to-hit operational measures are being sunset. Between 25% and 41% of plans could see a reduction of at least one-half Star. A recent court decision in favor of Clover Health could reduce the number of measures from 34 to as low as 16. A judge said CMS does not have the statutory authority to include some measures and didn't follow procedures when including others. The case is likely to be stayed or overturned. If not, Congress will likely intervene as value-based payments are key to healthcare reform. Congress could make further changes, including making the program budget neutral and gauging quality at the local level. MedPAC, former CMS officials, and the influential Paragon Health Institute are all pushing for further reforms. MA plans need to redouble their efforts as Stars gets more complex and rigorous. The 2025 and Q1 1026 results show plans enforcing discipline and signaling conservative guidance. This has sent stocks soaring from recent lows. Plans' discipline meant retrenchment -- reducing benefits and choice as well as higher premiums and cost-sharing for consumers in all lines of business. But the recovery is not a slam dunk. Insurers will see tighter rates, further prior auth reform, and less enrollment due to Medicaid and Exchange changes. Vertically integrated insurers likely face less risks, while those with greater Medicaid and Exchange enrollment are more vulnerable. Consumers are paying more because somebody has to absorb the higher structural cost burden in healthcare. And over the next several years, we are likely to continue seeing a healthcare system defined less by expansion and more by managed retrenchment.
