E18 - The Bipolar Brain on Overdrive: Dopamine, Serotonin & Circuits

Want to let us know something or ask a question? Send us a text! [https://www.buzzsprout.com/2185312/fan_mail/new] In this episode of The Y in Psychiatry, Dr. Amayo, Dr. V Handratta, and Dr. Nguyen take bipolar disorder off the DSM page and into your brain, but through your ears. They explain through tracing how dopamine and serotonin imbalances map onto psychomotor agitation, depression, and that “too much energy” feel of mania. They fly through the basal ganglia–thalamus–sensorimotor loop, the ventrolateral and ventromedial prefrontal cortex, the amygdala, and the superior frontal gyrus to show how disrupted circuits drive impulsivity, hyperemotionality, and goal-directed behavior. Say that 10 times. Along the way, they drop practical mnemonics, callbacks to network neuroscience, and framing you can use at the bedside or on rounds—no fMRI in clinic required. https://www.nguyenindoubt.com/theyinpsych https://feeds.buzzsprout.com/2185312.rss

Want to let us know something or ask a question? Send us a text! [https://www.buzzsprout.com/2185312/fan_mail/new] We hop aboard the Bipolar Express to unpack one of the messiest puzzles in psych: why diagnosing Bipolar Disorder sometimes feels like trying to solve a Rubik’s cube — when there could also be ADHD, anxiety, personality disorders, substance use, sleep-issues, or even childhood trauma riding shotgun. We'll walk you through the common—and often overlapping—comorbidities, explore how neurobiology and development muddy the waters, and talk real-world strategies for disentangling symptoms and giving clear, safe, and effective care. https://www.nguyenindoubt.com/theyinpsych https://feeds.buzzsprout.com/2185312.rss

E15 - Late Life Depression: When the Golden Years Turn Grey (and the SSRIs Don't Shine)

Want to let us know something or ask a question? Send us a text! [https://www.buzzsprout.com/2185312/fan_mail/new] We close with a deep dive into late-life depression, where we explore why growing older can sometimes mean growing sadder. Dr. Handratta and Dr. Amayo uncover the four key culprits: executive dysfunction (DED, not a good acronym!), vascular woes, inflammatory fires, and amyloid beta blues. We’ll discuss why SSRIs might not be the golden ticket and what alternatives, like dopamine agonists, rTMS, and even ECT, can brighten those grey years. Plus, we'll ponder if your patient is depressed, or if their brain is just throwing a vascular tantrum. Tune in for insights that are as informative as they are, well, hopefully not too depressing. Four Key Theories: Depression Executive Dysfunction Syndrome (DED): Frontal-striatal white matter tract issues, poor SSRI response, dopamine agonist consideration. Vascular Causes: Hypertension, diabetes, multi-infarct dementia, rTMS, ARBs, Calcium channel blockers. Inflammation: Indoleamine 2 3-dioxygenase, tryptophan depletion, CRP levels, TNF alpha antagonist. Amyloid Beta Deposits: Hippocampal involvement, Alzheimer's risk, cholinesterase inhibitors. Treatment Options: SSRIs (often with augmentation), pramipexole, rTMS, ECT (highly effective), and addressing underlying medical conditions. Differentiating Diagnoses: Differentiating between depression, vascular dementia, and other neurocognitive disorders. ECT: Most effective treatment for late life depression. https://www.nguyenindoubt.com/theyinpsych https://feeds.buzzsprout.com/2185312.rss

E14 - Liver and Let Die: Navigating Antidepressant Use in Cirrhosis

Want to let us know something or ask a question? Send us a text! [https://www.buzzsprout.com/2185312/fan_mail/new] In this enlightening episode, the squad unravels the intricate tango between the liver and psychiatric medications, focusing on patients with cirrhosis. Discussing 1st-pass metabolism, phase 1 and 2 metabolisms, plasma protein binding, fluid overload, and the implications of these processes on medication efficacy and toxicity. We know you liver this stuff. Our hopes and dreams by the end of this episode for you: Understand the significance of 1st-pass metabolism in the context of liver disease, particularly cirrhosis. Analyze the role of phase 1 and phase 2 metabolisms, and how they influence the efficacy of medications like antidepressants. Discover how plasma protein binding and fluid overload can affect the concentration of medications in the system and potentially cause toxicity. Learn about the specific antidepressants that go through 1st-pass metabolism, have high plasma protein binding, are broken down by cytochrome, and are affected by fluid overload. Discuss safer antidepressant options for patients with cirrhosis and the importance of monitoring and adjusting dosage. https://www.nguyenindoubt.com/theyinpsych https://feeds.buzzsprout.com/2185312.rss
